INSERT する前に存在確認を行うことで、UPDATE を無くして既存の問題の情報（contest_id 等）が ADT によって上書きされないようにした。

INSERT 前に存在確認を行い、既存の problem の metadata（contest_id 等）が ADT によって上書きされないようにしました。
変更点: problems の upsert を存在チェックベースに変更。無駄な UPDATE SET id = id を削除。 est_crawl_adt_problems を追加し回帰を防止。upsert_problems に ADT 対策コメントを追記。
