Drop sbt-coveralls (no sbt2 build), upload coverage via coverallsapp/github-action. Add scalafmt.

Drops sbt-coveralls (no sbt2 build), uploads coverage via coverallsapp/github-action instead. Adds scalafmt. Fixes root/module output dir clash and disables -Wnonunit-statement for tests.
