[WIP] Remove unused components from FHIRLab and document services

Remove all unsed components from FHIRLab (only to keep: HAPI, Ontoserver, Snowstorm, Bahmni, Aidbox, BedaEMR, SMILE CDR).

Document final list of running services and exposed ports

Confirm existing setup of 2 instances of HAPI (please make 1 default BAse R4 and other with PH Core)

1. Publish FHIR end-point and capability statement clearly reflecting actual behaviour

2. Support Validation against one IG
   Confirm existing setup of Ontoserver

3. SNOMED Intl should be available

4. SNOMED GPS should be available

5. LOINC should be available

6. Publish FHIR end-point
   Confirm exiting setup of SnowStorm

7. SNOMED GPS should be available

8. Publish FHIR end-point
   Configure HAPI FHIR to use Ontoserver for validation
   Demonstrate $expand and $validate-code

9. GitHub Readme; with Basic diagram of all component and Inventory of all services/components and end-points.

10. User friendly name for FHIR Server w.r.t IG.
