[codex] Update Hermes and plugins for unified stores

• updates Hermes generated plugin behavior for unified TraceDecay stores
• removes legacy migration module and legacy skill install paths
• refreshes Codex/Cursor plugin bundles and Hermes bridge tests

• cargo test --workspace on the top of the stack
